Inpatient case files, 1886-1960, 1970-1977.

Currently under the supervision of the Dept. of Mental Health, Westborough State Hospital and its predecessors have been responsible for providing residential care and treatment to mentally ill persons in Massachusetts since 1886--prior to 1939 specializing in homeopathic medicine and since then serving more generally the area south and west of Boston. Inpatient case files are created to provide a record of the commitment, diagnosis, treatment, and care of patients admitted to the hospital. Files may contain some or all of the following: admissions record; records of physical and mental examinations; clinical and medical records and charts; doctor's order sheets for medication; escape reports; injury reports; chronological notes on patient's condition and progress; death certificate and mortuary report; correspondence with interested parties concerning patient. Photograph of patient is generally attached to outside of case folder. Information may include name, registration no., date of admission and diagnosis at time of admission, how committed, date of discharge and condition at discharge, cause of death, and personal and family history.
